Personal Plan Specialist
This FULL TIME position would be responsible to
complete initial assessments, initial plan and coordinate
start of services. Monitor and review department
incident reports. Be responsible to review all
plans and supporting documentation for Community
Living Department and develop and up date the
Individual Personal Plan. Communicate regularly with
all team members, including individuals, guardians and
outside agencies. Be responsible for assigned QA/QI
duties. Participate in on-call rotation. Experience preferred.

Home Care
PART TIME and FULL TIME DIRECT CARE MANAGERS,
for 2nd and 3rd shifts in both Clinton, DeWitt
and some surrounding areas. You would be working in a
home setting, helping with all daily living skills and
social interaction with our individuals with disabilities.
This would include meal preparation, menu planning,
laundry skills, social skills, medication management,
personal hygiene skills and community outings. This is
very rewarding work and a great job for students who
are going into any type of social work.
PART TIME HOME HEALTH AIDES for flexible hours
which include some early AM’s, afternoons and early
evenings in Clinton and DeWitt. As a Home Health
Aide you provide personal care related to services in
the home under the direction of the supervising nurse
and Home Health Administrator. HHA’s help client
maintain good personal hygiene, and assist in maintaining
a healthful, safe environment. Help to plan and
prepare nutritious meals. Assist with ambulation and
certain treatments as ordered by the physician and
supervised by our nurse. Attend state and federally
required in-services and trainings. Ability to bend,
reach, stoop and stand for long periods of time.
We offer $300 Sign on Bonus, $100 Referral Bonus,
paid trainings, mileage, tuition reimbursement, casual
dress and wage paid according to experience. Part
time would be 15 to 25 hours per week, or more hours
if you prefer. We will work around your school schedule.
Interested individuals must have a valid driver’s
license, good driving record, a reliable automobile and
current insurance. You must also have the ability to
lift 50 pounds using proper lifting techniques. We will
train.
